Once you have reviewed the information below, please fill out the form below as best as possible so that we can arrange interviews. Your intake will be forwarded to our Intake Cooridnator who will handle your file and contact you shortly.
INTERVIEWS Our Intake Coordinator will reach out as soon as they receive the form with more information and interview details. Once they have setup the zoom interviews, you will receive the dates, times and zoom link for the professionals you will be interviewing. You will also receive a sample list of questions to help you through the interview process. Interviews are scheduled for 30 minutes with each professional and are recorded for internal use only. Once you have completed your interviews and have selected the professional you would like to work with, let the Intake Coordinator know so they can obtain all the details to have the agreement created and place your selected professional on hold.
Things to know once you have completed the interview Once you have completed your interviews and selected your professional, a contract will be sent to you with the specific details. The agreement will also include: - Deposit Amount (50% of the full agreement upon signing, remaining 50% 6 weeks before due date). Preferred method of payment is e-transfer. -Specific Inclusions - most birth packages include 2 prenatal visits (approx 1-2 hours each) and 1 postpartum followup phone call -Back up support (if requested and available we will be able to set this up) -Alternative options for C-Sections (For scheduled C-Sections you will have the option to have the professional support you during the c-section or receive a partial refund. For C-sections after labouring, there is no refund)
